Further to our email sent July 1, 2026, you have been randomly selected for Quality Assurance (QA) review as part of the Continuing Competence Program (CCP) requirement. This review ensures registrants’ compliance with the CRNA’s standards.

Form Submission 

Please submit your 2025 CCP self-directed learning plan here. This form is modelled after the Self-Directed Learning Plan Worksheet so you can easily copy and paste your answers in a familiar format. We suggest that you read each section carefully and keep your answers concise. The form should take an average of 10-20 minutes to complete.

All submissions are due by Sept. 30, 2026. Once your learning plan is received and reviewed, we may follow up with clarifying questions or request further documentation.
