Thank you for confirming your preference to submit your 2025 CCP self-directed learning plan via email.

Your 2025 practice year (Oct. 1, 2024 - Sept. 30, 2025) submission must include:

• Your topic of focus for your learning
• Your primary domain of practice (Clinical, Research, Education, or Administration)
• A rationale for the topic of your learning
• The corresponding practice standard (1.1-4.8 found on pages 6-9 in Practice Standards for Registrants)
• Completed learning activity (course, seminar, resource)
• One key takeaway for professional implementation

Documentation we do not require to complete your 2024 CCP QA review: 

• Receipts  
• Certificates 
• Attendance records 
• Articles 

All submissions are due by Sept. 30, 2026. Once your learning plan is received and reviewed, we may follow up with clarifying questions or request further documentation.
